The purpose of this application is to allow us to learn about you and confirm that you are eligible to participate in our program. The information you provide helps us connect you with the right adventures, so make sure you indicate which ones you find most interesting.
If issued one; MUST contain blocks 23-30
If you are a retired firefighter, police officer, or EMS, please attach copies of your retirement certificate/paperwork.
If you are disabled and have a VA disability rating, submit a copy of your ITEMIZED VA Disability Compensation Award letter showing each of your service connected conditions.
If you were wounded in action, and your Purple Heart is not on your DD 214, include a copy of your certificate or orders issuing the award.
